Day 5

Subjective: Mrs. X reports that today was her first day back to work and feels she over did it. She could

feel her shoulder twitching throughout the day. She wore her sling during the day but knows she was

guarding her shoulder more during the day. Has more discomfort that usual.

Pain – 5

Objective: Soft tissue mobilization performed to decrease pain and guarding, Completed therapeutic

exercises. Applied compression and ice to decrease swelling.

Assessment: Mrs. X presents today with an acute flare up of symptoms. Did not perform all exercises

today due to increased pain.
