Subjective: Mrs. X reported that she has been completing home exercises and it is getting easier to

move her arm.

Objective: Shoulder passive range of motion (PROM) flexion improved to 110 degrees. Right shoulder

PROM extension improved to 10 degrees. Provided patient verbal and tactile cues for proper technique.

Completed session with ice and compression to reduce swelling.

Assessment: Good adherence to home exercise program and improved PROM.

Plan: Follow up with patient’s doctor to determine which shoulder protocol to use. Massage of right

shoulder, PROM for extension and flexion.
